Immunizations / TB Screening
Only submit the documentation below if you are accepted into the program. Do not include it with your initial application. You may begin collecting your immunization records now and completing any missing vaccines.
- Your name and date of birth
- The vaccine or test given
- Date administered
- The clinic or medical staff's name, seal, letterhead, or signature

Tetanus (Tdap)
Current Tdap — must be less than 10 years old.

MMR (Measles, Mumps & Rubella)
One of the following:
- Documented series of two vaccinations, OR
- Positive titer for each of the three diseases

Varicella (Chickenpox)
One of the following:
- 2 Varicella shots, OR
- Positive titer, OR
- "History of disease" on letterhead reported by a physician

Flu Shot
Required each year. Once you are accepted into the program you will be given instructions on when to get your yearly flu shot.

Hepatitis B
One of the following:
- Proof of a complete Hepatitis B vaccine series (available as a 3-shot or 2-shot series — confirm which series you are receiving so you know when it is complete), OR
- Positive titer showing immunity — the required titer is called Hepatitis B Virus Surface Antibodies, Qualitative

Tuberculosis (TB) Screening
Must have been completed within one year of classes starting. Any of the following will be accepted:

COVID-19
Not currently required. However, if you have a documented vaccine series, we ask that you submit proof if accepted into the program.
- Pfizer or Moderna: series of two vaccinations
- Johnson & Johnson: one vaccination
Submitting proof will maximize the number of clinical partner sites you are eligible to attend in the event of a public health concern or policy change.
All immunization requirements above must be kept current throughout the program.
CPR Certification
Accepted applicants must provide written documentation of successful completion of an American Heart Association CPR course designed for healthcare professionals — Basic Life Support for Healthcare Professionals (BLS).
- Your CPR certification must remain current throughout the program.
- This documentation is only required if you are accepted into the program. Do not submit it with your application.
- You will be given specific information about this requirement if you are accepted.
Drug Screening
You must pass a pre-employment drug screen.
- You will be given specific instructions about this process if you are accepted into the program.
- Do not submit anything related to drug screening with your application.
