About your sonogram
- There is no charge for this sonogram.
- Your images will be projected on a closed circuit TV for you to watch when available. You may bring a flash drive with you to take home images.
- Your visit will take approximately one hour. You must be at least 18 years old.
- A student will spend approximately 45 minutes to 1 hour scanning you.
- The instructor may spend an additional 15 minutes scanning you.
- Please do not bring food or drink to the lab.
Eligibility requirements
To be eligible for this sonogram, you must meet the following requirements.
Your physician or their designee is required to complete and sign this form giving us permission to do this procedure. You should bring this signed form with you to the ultrasound lab.
Important: We will not do the sonogram without this signed form.

Physician consent
To be completed and signed by the patient’s physician or their designee. Fields can be filled in on screen before printing; the signature must be handwritten on the printed form.
